HHAeXchange+ Mobile App Goal Tracking Feature

The HHAeXchange Goal Tracking module is an Enterprise add-on designed specifically for Intellectual and Developmental Disabilities (IDD) and Developmental Disabilities Division (DDD) service providers. It helps agencies operationalize person-centered planning, deliver goal-based services, and monitor progress.

Goal Tracking supports the full lifecycle of goal management: care plan creation and goal definition, visit documentation, progress tracking, and supervisory review.

How to Log Goal Progress During a Visit

This articles teaches you how to document a person's goal progress during a scheduled visit - either on the web or in the HHAeXchange+ Mobile App.

Why This Matters

When you log goal progress during a visit, your notes go straight to your coordinator and become part of that person's ongoing care record. It only takes a few minutes and helps the whole care team see how someone is doing over time.

Option 1: Using the HHAeXchange+ Mobile App

  1. Open your scheduled visit.

    1. In the HHAeXchange+ Mobile App, go to your assigned visit for today.

  2. Select Goal Tracking.

    1. Inside the visit, tap the Goals tile.

  3. Review the goals.

    1. You will see the active goals for this person, based on their care plan.

  4. Mark each goal.

    1. For each goal, select ✓ or ‘X’ to indicate whether the patient worked on each goal.

  5. Add notes.

    1. If asked, enter a short note, reason(s) or prompts - this may vary depending on your agency's template.

  6. Save.

    1. Tap Save. Your note is sent to your coordinator automatically.

Option 2: Using the Web (Visit Info Tab)

If you are documenting from a computer instead of the app - for example, in a classroom or community setting - use these steps instead.

  1. Open the visit.

    1. Find today's visit by navigating to Schedule > select the Visit > open the Visit Info tab.

  2. Scroll to Goal Notes.

    1. You will see the goals set up for this person.

  3. Select a goal.

    1. Click the checkmark next to the goal you are documenting. This opens the note form.

  4. Fill in the note.

    1. Complete the fields - select ✓ or ‘X’ to indicate whether the patient worked on each goal. If asked, enter a short note, reason(s) or prompts - this may vary depending on your agency's template.

  5. Save.

    1. Save the note. It is now sent to your coordinator for review.

If something looks off: If you do not see the goal or plan you're expecting on a visit, let your coordinator know - it may just need to be linked to the correct visit type.

Quick Tips

  • The app and the web show the same note - use whichever is easier where you're working.

  • Fill in notes as soon as you can after the start of a visit, while details are fresh.

  • If a field is required, you won't be able to save or clock-out until it's completed.
