HHAeXchange+ Mobile App Questionnaires Feature

Use the Questionnaires feature of the Mobile App to answer daily questions about the Patient’s health and well-being. Questionnaires are questions to the Caregiver that evaluate patient health. For example, Is the patient eating less than usual?. This feature is enabled by the Agency.

To complete Questionnaires for a Patient Visit,

1. Clock in to the Visit, and then tap the Questionnaires tile. To access Questionnaires after the visit go to Schedule > Visit > Visit Details.

   

2. On the Questionnaires page, answer all required Patient Care Questions. Provide Additional Info for any answer, if requested.

   

3. After you answer all required Patient Care Questions, tap Submit.

   If the mobile app is off-line when you clock out, Questionnaires do not appear. If the mobile app goes off-line while you are answering questions, answers cannot be saved and a warning displays.
