Add, Edit, or Delete a Cohort

Complete the following steps to manually add a new Cohort to Care Insights.

On the right area of the Cohorts page under Provider & Caregiver > Care Insights, Cohorts and click Add Cohort.

In the New Care Insights Cohort page that appears, enter a Cohort Name and Cohort Description.

(Optional) Select Deactivate members absent in the file to deactivate any Member not in the file to be uploaded.

Under Cohort Data to be Imported, click Choose File. In the window that appears, select the .csv file from your computer.

Click Saveto finalize. The new Cohort appears in the Cohorts tab with the Members whose Admission IDs were in the .csv file.
