Care Insights Overview

Care Insights documentation describes how to use Care Insights, found on the Provider & Caregiver menu, which integrates with the Payer Portal and allows Payers to collect information about Member health from within the home.

With Care Insights, Payers create questions to identify leading risk indicators that could end in adverse and costly events such as hospitalizations, emergency room visits, or skilled nursing facility admissions. Questions are assigned to Cohorts—and then appear on HHAeXchange mobile apps at Caregiver Clock-Out, enabling targeted health evaluations at the point of care. Alerts in Care Insights then surface critical information to Member care teams.
