Plan of Care
A Plan of Care (POC) is the list of duties performed by a Caregiver when providing services to a Member. A POC defines which duties will be performed, on what days of the week, and during which date range.
To access POC functionality, from the Member Account page go to Member > Search > search for and select a Member, click Plan Of Care. The Plan of Care (POC) page appears.

On the right side of the Plan Of Care page under Member > Search, search for and select a Member, Plan of Care, and then click Add POC.

On the upper area of the New POC window that appears, enter POC details including the Start Date (required), and optionally, the Stop Date, the Shift, and Notes about the POC.

On the lower area of the New POC window, choose the Duties to be performed by the Caregiver by selecting one of the following:

As Needed - Select this box if the duty is to be performed when the Member requests it. If necessary, also add the Min-Max Times a Week that the Duty is to be performed.

Days of the Week - Select the specific days of the week that the Duty is to be performed.

Min-Max Times a Week - Enter the minimum and maximum times of the week that the Duty is to be performed, regardless of the specific day of the week.

In the Instruction column, add any additional instructions for each Duty.

Click Save.

On the Plan Of Care (POC) page under Member > Search > search for and select a Member > Plan of Care, in the Action column, click the Ellipsis ... next to the POC, and then select Edit.
Payers cannot edit POCs created by Providers. These POCs appear as read only.

In the window that displays, make changes to the POC as necessary.

Click Save to finish editing the POC.
