Care Insights Questions Tab

To access Care Insights Questions, click the Questions tab at the top left of the Care Insights page under Provider & Caregiver > Care Insights.

On the Questions tab, a user can add and edit Yes or No questions to be assigned to one or more Cohorts, which then appear in HHAeXchange mobile apps, enabling Caregivers to evaluate Members at the point of care. Questions are customizable, can be mandatory or optional, can appear in defined recurrence patterns, and can enable additional free text fields where additional information is added.

Questions need not be verbal interactions with Members. They can also be evaluations of Member health. For example, a question might ask a Caregiver: Do you observe any unsanitary conditions in or around the home?

Alternate language translations of Care Insights Questions can be entered in any of the languages supported by the HHAeXchange+ Mobile App (Unified). These translations is available to Caregivers in the HHAeXchange+ Mobile App (Unified).

Alternate language Care Insights Questions is an exclusive feature of the new HHAeXchange+ Mobile App (Unified) being rolled out to Agencies over time. This feature is not available on the classic HHAeXchange Mobile App.
